  1. This contribution is made from organizational funds under the authority of the identified donor organization and is not drawn on the account of, or made on behalf of or in the name of, any other individual or entity.
  2. The identified donor organization for this contribution is a domestic entity formed under the laws of the United States and is not directed, controlled or owned by any foreign individual or entity.
  3. This organizational contribution will not be reimbursed by another person.

Contributions are not tax deductible as charitable contributions for federal income tax purposes. State law requires us to obtain and report: the name, mailing address, occupation and name of employer for each individual whose contributions aggregate in excess of $100 per calendar year; and the organizational name and mailing address for each entity whose contributions aggregate in excess of $100 per calendar year. Contributions by foreign nationals, entities directed, controlled or owned by foreign nationals, and minors under the age of 18 are prohibited.
